Car jerks or hesitates when you accelerate
Jerking under acceleration means the engine is losing combustion at exactly the moment you are asking most of it. Three systems can cause it, spark, fuel and air, and the way it behaves tells you which one to look at first. A tune-up fixes a good share of them. The rest are worth finding before you spend on parts.

Whether it happens under load or off load splits the list almost in half. Ignition faults show up when you demand power. Lean and vacuum faults show up when you are barely touching the pedal.
Why load is the most useful clue you have
An ignition coil has to push a spark across the plug gap, and the harder the engine is working the more resistance that spark has to overcome. Cylinder pressure rises under load, and a weak coil that manages fine at cruise simply cannot jump the gap when you floor it.
So a car that pulls smoothly at light throttle and stutters going up a hill or merging onto a highway is describing an ignition fault almost word for word. Worn plugs with a gap that has opened up over 80,000 miles do the same thing, for the same reason.
The opposite pattern points elsewhere. If it stutters at a steady 30 mph with your foot barely on the pedal and cleans up the moment you accelerate properly, the engine is running lean at low load. That is vacuum leak behaviour. At light throttle the unmetered air is a large share of what is going in, and opening the throttle drowns it out.
Neither of those is something a scan tool will tell you on its own, which is why we road test rather than just reading codes in your driveway.
Fuel delivery, and the fault that gets worse as you drive
A fuel pump that is wearing out does not usually fail cleanly. It gets weak when it gets hot, which means the car is fine for the first ten minutes and starts hesitating after twenty. If the jerking is worse at the end of a drive than the start, that pattern is worth taking seriously.
A clogged fuel filter behaves similarly but is more consistent, and it is far cheaper. On cars that still have a serviceable filter it is one of the first things worth ruling out, particularly on a car with high mileage and no service history.
Dirty injectors are the third possibility and they tend to produce a rougher, more scattered hesitation rather than a clean cut-out. Direct injection engines get carbon build-up on the intake valves as well, which upsets airflow into the cylinder and shows up as exactly this kind of stumble.
The way to separate these is pressure, measured on a gauge while the engine is under load, not guessed at from a code. A pump that holds 55 psi at idle and drops to 30 under acceleration has told you everything you need to know.
Sensors that lie, and why the computer believes them
A mass airflow sensor measures the air entering the engine so the computer knows how much fuel to add. When it gets contaminated it under-reports, the computer under-fuels, and the engine goes lean and stumbles precisely when you accelerate and airflow spikes.
This is one of the more common causes of hesitation on cars past 60,000 miles, and it often sets no code at all, because the sensor is not failed, it is just reading low. The computer has no way to know it is being lied to. It only sees a number.
Fuel trims are how we catch it. The computer records how much it is having to correct the mixture, and a long-term trim sitting well positive says the engine is being starved of fuel somewhere. That number plus a road test narrows it to the sensor, a vacuum leak or a fuel supply problem quickly.
A failing throttle position sensor or an oxygen sensor drifting slow can produce the same complaint. None of them announce themselves. All of them show up in live data if somebody looks.
When it is not the engine at all
Worth saying plainly, because it saves people money. If the jerk happens at one particular speed, or only when the car changes gear, or feels more like a shudder through the whole car than a stumble in the engine, you may be looking at the transmission rather than combustion.
A torque converter clutch that shudders as it locks up produces a very distinctive light vibration at around 40 to 50 mph, often mistaken for a misfire. Worn transmission fluid causes it more often than a failed part does.
Equally, a driveshaft or CV joint problem gives a vibration that rises with road speed and does not care what the engine is doing. Lift off the accelerator and coast. If the sensation continues while the engine is doing nothing, it is not the engine.

The flashing light rule
One thing overrides everything else on this page. If the check engine light is flashing while the car jerks, stop driving it.
A steady light means the computer has stored a fault. A flashing light means a misfire is happening right now and is bad enough that unburned fuel is going down the exhaust. Catalytic converters run hot and are not designed to burn raw fuel. Doing it for a few miles can melt the substrate inside, and that turns a set of ignition coils into a job costing many times more.
We get called to cars where the light had been flashing for a week because it still drove. It always drives, right up until the converter blocks and it does not.

Common questions
Why does my car jerk when I accelerate but drive fine otherwise?
Because acceleration is when the engine is under the most cylinder pressure, and that is when a weak spark fails to jump the plug gap. Worn plugs and tired ignition coils are the two usual causes, and both are ordinary wear items. It is also when fuel demand peaks, so a weak pump or a clogged filter shows up at exactly the same moment.
Can a dirty air filter cause jerking?
It can contribute, but it rarely acts alone. A badly clogged filter starves the engine of air under load and makes it feel flat rather than jerky. If the filter has never been changed it is worth doing regardless, because it is cheap and the engine wants it. If replacing it does not change the jerking, the cause is further in.

Should I try fuel injector cleaner first?
It is cheap and it does no harm, so if the hesitation is mild and gradual it is a reasonable first try. What it will not do is fix a weak coil, a vacuum leak, a contaminated airflow sensor or a failing pump, which between them account for most cases. If a bottle does not change anything, stop buying bottles and get it read.
The car jerks and the check engine light is flashing. What should I do?
Park it. A flashing light means an active misfire severe enough to be sending unburned fuel into the catalytic converter, and continuing to drive risks turning a set of coils into a converter replacement.
